A Case of Microgranular Acute Promyelocytic Leukemia




Abstract
Microgranular acute promyelocytic leukemia microgranular(APL) has been described as
a variant of APL in which the majority of the leukemic cells have a nuclear
configuration resembling monocytes and are avoid of heavy granulation and multiple
Auer rods. This microgranular APL may be confused with other types of acute
nonlymphocytic leukemia, especially monocytic leukemia. However, microgranular APL
follows a similar course to APL, including disseminated intravascular coagulation.
We experienced a case of a 42 yrs old female who has leukocytosis including 74%
leukemic cells. The cytochemical reactions of these cells showed strong positive for
myeloperxidase, diffuse weak positive for PAS and negative for non-specific esterase
stain. The chromosomal study was 46, XX, inv(9) (p21 q13) and leukemic marker study
revealed positive in 39% of leukemic cells for MY9, in 31% for MY7 and in 54% of I©ü.
